First, click on the form link below and print the admission checklist. This will tell you what will take place and what is required for each step of the intake process.

Requirements for Admission Checklist

The following forms are required to begin the intake process:

	Eagle Ranch Application
	Eagle Ranch Child Recommendation
	Criteria for Psychological Evaluation

The following forms are required after the child has been accepted, but must be completed before the child enters the program.  It is very important that all forms are dated within 30 days of the child's arrival at Eagle Ranch.

	General Physical
	Dental Checkup
	Urinalysis
	CBC Test
